Properly filling out your OJT form and faxing it to Sacramento is a monthly requirement. Classes are given during the student's orientation on how to properly fill the form out. Use this link to see an example of two computerized OJT forms.

Computerized OJT form services are available to our students. Within the next year we expect to have the forms available online as well. With these forms online, a student will be able to turn in their activities daily and at the end of the month print out the form to be signed and then faxed to Sacramento.
